If you can’t show it to the insurance company, they will not cover you expenses. What if you lose a receipt? Well, you will have a problem - that’s for sure. However, there is a way out. You will have to go back to the doctor’s office and ask to him to give out a signed document, which proves your previous visit. The doctor will check his journal and give you a new receipt.
- To fill out a claim form, first you need to get it from your health insurance company. All claim forms are usually easy to fill in. You will put down your insurance information, the reason of going to a doctor, and the person you want the payment to be made to. If there is any additional information required in order to get a refund, it will be noted in the claim form. So read it carefully and pay close attention to all the details.
- Make copies of the filled claim form.
